Firstly, look at the air ducts themselves and see if there is dust, debris, and mold. Buildup of such indicates that there’s an issue worth inspecting.
If there are dark spots on walls near the vents, that also likely comes from dust. Bigger dark spots on a wall that also look fuzzy might even be mold coming from a moisture source somewhere in that area.
HVAC system malfunctioning, like not heating and cooling properly or with less airflow, might also be a sign that you need to have your air ducts cleaned.
Bad air quality all around is a red flag. If your system runs and it smells musty, someone in the room starts coughing or sneezing, or the smell even triggers allergies, that’s a problem.
And last but not least, pests or signs of pests, like nestings.
